Survey

The survey linked to from this page is to help FOML publish its' annual directory of Friends Groups throughout the state of Michigan. For your entry to be included in the next directory FOML must recieve a survey by March 31st, 2007.

If you completed a survey in previous years, please only complete fields that need to be updated, and if you add projects please indicate whether these are additions or replacements.

The following form has been designed for you to complete on your computer and then print out. Please note that if you are using Acrobat Reader, you will not be able to save the contents you enter in the form for later use, so please make a print off when you are finished completing the form.
